Difficult-to-treat depression (also known as treatment-resistant depression) affects 20–50% of people with depression. It causes profound distress, contributes to long-term disability, and places a heavy burden on individuals, services, and society.
Despite a growing evidence base across medication, psychological therapies, and neuromodulation (e.g. TMS, tDCS), many people still receive limited support, especially in primary care. Access to specialist services is patchy. Long-term symptoms are common. Recovery remains elusive for many.
System-level barriers, including service fragmentation, limited specialist provision, and stigma, prevent timely and effective treatment.
