Visitor Screening Notice:

1. Have you recently travelled to an area with known local spread of coronavirus? (Yes / No)

2. Have you come into close contact with someone who has a coronavirus symptoms or a confirmed coronavirus diagnosis in the last 14 days? (Yes / No)

3. Have you been in close contact with anyone who has travelled to an area with known local spread of coronavirus within the last 14 days? (Yes / No)

4. Have you experienced any cold or flu-like symptoms in the last 14 days (to include fever, cough, sore throat, respiratory illness, difficulty breathing)? (Yes / No)

• If the answer to any of the above is "Yes", please do not continue with the booking. This is to ensure the safety of all those who will be attending the service. However, if the answer to all of the above is "No", please proceed to register.

• A temperature check will be conducted for everyone who arrives on site before entry. If the result is within normal levels access will be granted. Otherwise you will be asked to reschedule your visit.

• If after registering for the service your circumstances have changed so that any of the responses above becomes a “Yes”, please do not attend the service. You may contact the organizer on 02072720449 to update accordingly.
