Transforming Diabetes Care Services in Croydon

If you have type 1 or 2 diabetes and are a Croydon resident, NHS Croydon Clinical Commissioning Group (CCG) want to hear from you about your experiences of the diabetes services you currently use.
Croydon CCG plan and buy most of the Health services on behalf of Croydon residents, including the services you use at Croydon University Hospital and the intermediate diabetes service run by Bromley Healthcare.
We are currently looking at how we provide specialist services for people with diabetes. This event is for everyone with an interest in diabetes care locally, whether as a patient, carer or someone who supports people with diabetes.
This meeting is your opportunity to:
Meet together with other patients, carers, providers and commissioners to talk about the development of ideas around improving diabetes care for Croydon residents.
Give us your feedback on the right care for Type 1 & Type 2 diabetics including care in the right place at the right time.
Improve the experience of patients.
Receive information and advice regarding managing your diabetes.
This is the start of the conversation about how we develop services which meet the needs of patients, families and carers for the future.
