Diabetes UK, with support from Roche Diabetes Care, would like to invite North West CCG diabetes and long-term condition commissioning managers, clinical leads and all those involved in diabetes service re-design, to join us for our third interactive and educational commissioning forum

10:15 Diabetes Service Redesign to reduce unplanned admissions: the NHS England Perspective
Partha Kar, Consultant Diabetologist at Portsmouth Hospitals NHS Trust and Associate National Clinical Director for Diabetes, is the pioneer of the Portsmouth Super Six model and he presents the impact of the model on diabetes care, hospital admissions and NHS budgets. Partha will also offer reflections on the state of diabetes commissioning and service provision at a national level.

13:15 Showcasing good practice from across the North West - A Toolkit to Review Structured Patient Education
Dr Quincey Chukka of Warrington CCG and Charlotte Hall of the Innovation Agency will share the results of a Diabetes UK project to develop a practical, detailed toolkit that helped Warrington CCG to review the commissioning and provision of structured education in Warrington. This toolkit will be available in e-form for all delegates to take away.
Prescribing of Blood Glucose Testing Meters; decisions, decisions
Stephen Ryan and Yvonne Browne of Diabetes UK present an update on work to develop a decision tree to help CCG managers and clinicians decide on the right meters to prescribe for patients with diabetes who need to check their blood sugars. The guidance will be available in e-form for all delegates to take away and use.

14:30 Transformation Funding Transformation Funding: Who got what and what happens next?
Many delegates will already be familiar with the £42 million worth of transformation funding made available recently to improve diabetes services. Michele Cossey of NHS England will update delegates on the allocation of funds from the first round of bidding, and draw some key points to help those making bids in the future.
