Description

The population of Oldham, like many areas, has an increasing number of individuals that are working with a health condition. This event is primarily aimed at Oldham Health Professionals that are likely to work with these patients; GP’s, practice nurses, CCG commissioners, senior AHP’s, lung service, diabetes service, MSK service, stroke recovery service, drug and alcohol recovery services, mental health and wellbeing services etc.

The event will set the context for the scope of ambition needed to better support people with a health condition in Greater Manchester to remain in work. There will be a combination of presentations and interactive sessions to fully involve health professionals in discussions and ideas for service design and future intentions of work and health support.

Your views and experience will be used to inform the development of the GM Health and Social Care Partnership's plan to provide early help for people who are 'at risk' of falling out of work due to health conditions, as part of the prevention/early intervention priorities of the GM Population Health Plan.
