Module 1 : An Introduction to Personalised Care

Module 2 : Understanding the Role: Care Coordinators, Social Prescribers & Health and Wellbeing Coaches

This module explores how Care Coordinators collaborate with Social Prescribing Link Workers and Health and Wellbeing Coaches to deliver truly personalised, joined-up care.
