Description

The challenge of reducing DTOCs for NHS and Local Authorities

Delays in the effective discharge of patients consume huge resources in the NHS and Local Authorities. Delayed Transfer of Care days are up by 31% during 2013-15 reaching over 3.1 million and costing £800m per annum according to the National Audit Office.

In part, ineffective communication and a lack of collaboration underlies the challenge, which importantly, has more than just a financial cost. There is a substantial human cost too. Particularly, its impact on the elderly for whom longer detention in hospital than necessary can be damaging in terms of physical deterioration and exposure to other illness risks. NHS England Snapshot data shows that over 5,000 DToC patients are kept in hospital every day when they are medically fit to be elsewhere.

According to reasons given from NHS England's published data of all delayed cases -

• 17% are due to lack of NHS completing assessments
• 18% are due to lack of arrangements with other NHS organisations, and
• 17% are due to lack of arranging care packages at home.

Improved communication and collaboration of patient desired outcomes, needs and stakeholder responsibilities in supporting them could significantly improve the situation and yet, the Parliamentary and Health Service Ombudsman Julie Mellor said in late May:

'Poor planning, coordination and communication between hospital staff and between health and social care services are failing patients, compromising their safety and dignity'.

'Health and social care leaders must work harder to uncover why ten years of guidance to prevent unsafe discharge is not being followed, causing misery and distress for patients, families and carers'.
