This public workshop is being held as part of reviewing palliative care services.

Further to publishing the independent review of palliative care services earlier this year, work has been progressing across the four boroughs to look at how we address the challenges and inequalities highlighted in the report and develop services in line with the recommendations. Public workshops are taking place this Autumn, people from each borough will be invited to join health care professionals and staff from our services in a development session on the future model of care.

The aims of the workshops are to:• Bring patients, clinicians, commissioners and providers together collaboratively to discuss the theme of each workshop.• Share & understand patient & families experience of using local services.• Use this information to co-produce a new specialist palliative care model within the community for Brent, Hammersmith & Fulham, Kensington & Chelsea and Westminster.

WORKSHOP 1 - Theme for discussion ‘Access to services’– 19th September, Wembley, 2-5pm.

This workshop will look at how patients, families and carers access care – what this currently looks and feels like for patients, families and also clinicians.

WORKSHOP 2 – Theme for discussion ‘Care’ - 7th October, Hammersmith & Fulham, 5-8pm.

This workshop will look at what patients, families and carers want to receive as part of their care & treatment, what there is experience is now and what they feel it should be.

WORKSHOP 3 – Theme for discussion ‘Aftercare & bereavement’ - 24th October, North Kensington, 2-5pm.

The theme of this workshops is aftercare including the transfer of care to another setting, bereavement support for families and carers will also be discussed – what this looks like now and what patients and families feel it should look like.
