We are now one year into our ambitious ‘living well to the very end’ programme which aims to improve patients’ and families’ experiences in end-of-life care. The focus of the first year was to provide intensive training and coaching in the Patient and Family Centred Care (PFCC) methodology to eight teams across the South of England, in partnership with NHS England (South) and The Health Foundation.

Following this first year, on 23 February 2017 we are hosting a celebration event in London where our teams will present some of their wonderful projects. In addition a range of experts in end-of-life care will discuss the ongoing work to deliver the national strategy and others will share the latest evidence and best practice. The event will also provide the opportunity for delegates to learn about using the PFCC methodology in their own settings and to register their interest for the next cohort of the programme, which will launch in 2017.


Given the importance of patient-centred care at the end-of-life, and the impact on families when end-of-life care is truly patient-centred, attending this event, you will:

  • Understand the potential of the patient and family centred care (PFCC) methodology to improve the experiences of patients at the end of life and their families. You will hear from teams that have successfully applied the method in their own practice.

  • Learn how the PFCC method can be successfully applied to end-of-life care in generalist settings. You will learn the conditions for success, possible challenges and how these can be overcome.

  • Understand the next steps needed to apply patient-centred quality improvement methods to end-of-life care in your own setting. You will be able to register interest for the 2017 cohort of the programme.
