Description

Prescribing errors in general practices are an important and preventable cause of illness, hospitalisation and deaths. The PINCER trial was developed to study whether a pharmacist-led IT-based intervention could reduce medication error rates within the primary care setting. Published in the Lancet (Avery et al, Lancet 2012; 379:1310-1319), the PINCER Intervention was shown to be a clinically effective and cost-effective method for reducing a range of clinically important and commonly made medication errors in primary care. PINCER is now highlighted in the new GP contract framework. The data extraction for PINCER is newly improved.

These Dorset PINCER action learning sets will support the development of skills and processes in General Practice in Dorset to ensure common causes of prescribing errors are identified and resolved.

Practices are required to ensure that they attend both sessions


Session 1: will outline the background and evidence behind PINCER and outline how to access the indicators

Session 2: will focus on Root Cause Analysis Quality Improvement methodology to ensure that practices deliver the changes needed to improve medication safety.


Clinical representation is especially important for the second session.
