This PLI is aimed at GPs, Advanced Nurse Practitioners and Pharmacists

Aim:

Further improve quality and safety of prescribing

Learning Objectives:

* Understand the risks to patients of inappropriate polypharmacy

* Understand the benefit to patients of appropriate deprescribing and practical tools to support these conversations

* Be aware of tools and resources that can support appropriate deprescribing

Eg:

  • Patient Decision Aids

  • Local Guidelines

  • Local Pathways

  • Shared examples/learning

  • Data sources - eg open prescribing, polypharmacy reports

* Be able to apply the principles of patient centred care to achieve optimised prescribing

* Understand potential benefits of pharmacists working as part of the MDT

* Understand the support on offer from the different pharmacist professions (pharmacists and technicians working in Sheffield)

* Increased understanding of medication used in transgender

Polypharmacy and appropriate deprescribing: 14:30-15:15

  • share and highlight resources to support (PrescQIPP resources on Meds and prescribing section on intranet) - what is your 'go to tool?'

  • Shared decision making/Patient decision aids

  • Start at the beginning - have the conversation about depresribing at start so easier to engage conversation around deprescribing later down the line

  • Case study/ies, including patient outcomes

  • Polypharmacy award winner presentation

Please note the Medicines Management Team will be holding the Inaugural Prescribing Awards at this event - see below for more info and what you can do to enter!

You, your practice, neighbourhood or MDT are invited to share this work by entering the inaugural prescribing awards.

The three categories are;

  • Appropriate polypharmacy /deprescribing producing positive patient outcomes
  • Integration of pharmacy and other health care professionals to optimise patient outcomes

  • Medicines safety

The winner of each award will receive a certificate and the chance to share their good work with other membership practices

All applications should be submitted by Friday 16th January. Please submit to keeley.askew@nhs.net
