Polypharmacy ALS Foundation for HIOW pharmacists
Overview
Are you a pharmacist working in General Practice or a Neighborhood team and want to learn more about polypharmacy and medication reviews in older people?
The Polypharmacy Action Learning Sets have been shown to improve clinician's confidence in tackling medication reviews in people with multiple long term conditions. We have now adapted that course for pharmacists working in General Practice/Neighbourhood teams that are undertaking their IP training or are new to a prescribing role.
Join us to learn more about polypharmacy in older people and how to develop your role as an expert in structured medication reviews in primary care.
