Description

Building on the success of our previous conferences, the 2018 programme supports the sharing of learning to enable making positive change happen within our communities. 

There will be the opportunity to network and discuss new services with a wide range of colleagues and stakeholders as well as presentations on the following themes:

  • Making Collaborations Work
  • Working in Multi-disciplinary Teams
  • Managing Need & Demand

There are many examples of best practice that can be cited and a great deal of evidence is now available regarding interventions and innovations that work. Deploying these innovations consistently across HDUHB for the benefit of all will require a significant change in the way services are developed and delivered. If Primary Care is to maximise its potential in delivering care that is coordinated, accessible and proactive, then describing that service model clearly and providing compelling evidence demonstrating its impact on the wider system, will be an important first step. 

We are looking for partners to showcase their work to patients, primary, community and integrated care health professionals, partners and colleagues across Hywel Dda.  If you are a coordinator of a project or know someone who is and would like to showcase your work at this event please let us know by emailing;

The programme agenda and details will be sent out in the next few weeks via email but in the meantime please feel free to promote the event and the opportunity to present projects and initiatives with your colleagues and HOLD THE DATE.
