New Models of Care – Proactive Care Project
In the Beeston and Crossgates localities of Leeds South and East Clinical Commissioning Group a new model of care project is in development to test out the features of a proactive, integrated, patient centred carre model for people with multiple LTCs including CVD or COPD.
A new multidisciplinary team is in place in both localities comprising of GP, Geriatrician, Matron, Therapist and Health advisors, with provision for mental health and pharmacist input once the level of need has been identified. The model will focus on developing wellbeing plans in partnership with participants along with resilience plans that support better self-management of conditions, coordinates resources more effectively and uses community assets to better effect.
Earlier in the year many of you attended workshops to inform the early thinking and development of the project and that input has been incredibly valuable. We will be holding open meetings in the Beeston and Crossgates areas each month to give regular updates on progress, check the developing plans and to ensure your input is embedded as the project progresses. We hope these meetings will provide the space and time for us to regularly meet and explore ideas together.
We hope you can join us Tuesday 6th December from 15:00-17:00 for an update on progress so far. This will be an opportunity to ask the team questions and think about our work together moving forward.
