Description

As part of our continued drive to work in partnership and support residents in care homes receive safe, effective high quality care, we are letting you know about our plans to launch the “Red Bag” hospital transfer pathway across Bradford District and Craven in February 2018.

The aim of the Red Bag Pathway is to improve people’s experience when they need to go into hospital by helping to streamline and speed up transfers and information exchange between hospital, ambulance and care home settings.

The Red Bag Pathway involves care homes being given a Red Bag to use when a resident is being admitted to hospital. The Red Bag keeps important information about a care home resident in one easily identifiable and accessible place. This information includes; transfer documents, medication, paper work detailing information about the resident’s general health along and medical history. The bag also has room for personal belongings (such as clothes for day of discharge), and will remain with the patient while they are in hospital until they return to the care home.

The Red Bag Pathway is a simple but innovative concept and it will support care home, ambulance and hospital staff to determine the care a resident needs quickly and easily, overall improving patient experience and providing joined-up care.

This event is to celebrate the fantastic work and collaboration between lots of staff groups and launch the Red Bag Pathway across a number of care homes.
