About this event

  • 45 minutes

We know that personalised, well‐planned support services ("reablement") for up to six weeks can not only accelerate discharge rates, but reduce care dependency and hence prevent or delay the need for long‐term home care and other labour‐intensive community services. Remote care as part of a Hybrid care model can play an important enabling role - managing the risks, both during the reablement period, and in the months following service delivery, irrespective of whether the individual needs long‐term home care.

GenieConnect® aims to maintain and/or improve an individual's safety and independence, adding multiple touch points to enhance overall wellbeing, and prevent or reduce the risk of secondary health conditions.

In 45 minutes, you will learn

  • How remote care supports the reengineering of healthcare to deal with the needs of an older and more vulnerable population wishing to remain in their own homes.
  • A profile of appropriate users who would benefit from remote and hybrid care services.
  • How remote care can form part of a long-term support plan and personal budget dependent upon assessment and eligibility.

Who is this for?

  • Local Authorities and Councils
  • Care and Support Providers
