IMPORTANT VENUE INFORMATION: Access to the Sir Alf Ramsey Suite is from the Portman Road side of Ipswich Town Football club. Please enter the Executive Entrance - tip – the Sir Bobby Robson Statue is pointing to this entrance.

This SEND Workshop is part of a series of workshops in an annual programme to meet the SEND training needs of a multi-agency audience.

This workshop is for Health, Social care and Education practitioners.

In this workshop, there will be presentations by specialist speakers and a variety of activities to support the development of quality transitions for children, young people and adults (age 0-25)

The workshop will demonstrate person centred tools and emphasise keyworking.

The workshop scope will include all transitions, but will highlight the main issues within Moving to Adulthood.

Training Objectives- By the end of the session the participant will be able to:

Understand how the keyworking approach can smooth the transition process for children, young people and adults (age 0-25)

Plan for a more personalised experience of a child,young person/adult in transition planning

Show increased confidence by having person centred outcome conversations for transition with colleagues, children/adults and their families/carers
