We warmly invite registered nurses from any care home in the London region to register to attend Significant 7 train the trainer events taking place on the 11th January 13:30 to 17:00
The 'Significant 7' educational tool has been developed by the elderly care team at North East London Foundation Trust for use by nurses and carers in care homes to enable early identification of patient deterioration. The tool enables the carer or nurse to review the patient using a systematic approach to identify whether the patient needs routine or emergency review by a clinician.
The tool allows the carer or nurse to use a decision tree to identify signs of deterioration within seven domains, confusion; mood; pain; hydration; skin; breathing and toilet.
Initial analysis of the effectiveness of the tool in improving quality of care for patients appears promising. There was a marked reduction in A&E attendances for residents from the pilot care homes. Within the London Urgent and Emergency Care Improvement collaborative we have an enhanced health in care homes working group which includes senior STP representation, this working group is now engaging with independent care providers to implement the training to care homes using a phased approach.
