Introduction to the Six Steps End of Life Care Training Programme for Care Homes

Background

The Department of Health’s End of Life Care Strategy, published in 2008, emphasised the need to raise the quality of care provided to dying people and their loved ones in a variety of settings including care homes.

The strategy identified the need for care home staff to receive the training and support to ensure the provision of good end of life care.

It recommended more coordinated services which could see NHS and statutory social care authorities providing more support to care home staff and managers and their residents. Such support would reduce the number of unplanned hospital admissions, ensuring more people can die in their care home if that is their expressed preference (DOH 2008). One of the outcomes was the publication of a training programme for care homes.

The National Six Steps Programme was developed in partnership between Merseyside and Cheshire Cancer Network, Greater Manchester and Cheshire Cancer Network, and Cumbria and Lancashire End of Life Network with support from the National end of Life Care Programme. The programme aims to enhance end of life care through facilitating organisational change and supporting staff to develop their roles around end of life care.

The programme has been adapted by St Lukes Hospice Plymouth to provide an accredited training programme for care homes in Plymouth.

North Devon Hospice is committed to provision of end of life care to health care professionals in all settings to promote quality care for all patients. The Six Steps is a comprehensive training programme that offers care homes the opportunity to be competent and confident in provision of sensitive, compassionate end of life care for their residents.

What care homes will gain

This programme will enable you to meet Department of Health End of Life Care CQC & Dementia Quality Markers and support your staff to develop Core Competencies for End of Life Care.

On successful completion of the programme, Champions will receive an individual Certificate. An organisational Certificate of Accreditation will be awarded to the Care Home.

The Programme Content

  • 9 day-long taught sessions run over a period of 9 months, followed by a 3-month period to demonstrate achievement of appraisal criteria and CQC standards
  • Each session will cover a specific aspect of end of life care and provide time for individual portfolio development and tutor support

Care Home Requirements

  • Identify 2 champions, one with managerial responsibility
  • Champions should have NVQ/QCF Level 3 or above (or relevant experience) and will be able to influence change and champion end of life care in their workplace
  • Signed agreement to support 2 staff to undertake training
  • Support champions and enable them to attend all workshops
  • Share learning and implement change
  • Support the ongoing audit process
  • Support attendance of Care Home Forum / Six Steps Forum sessions

Requirements for the Champions

  • Completion of individual portfolios of evidence from each champion to support knowledge and skills to underpin excellence in end of life care provision
  • Completion of one Organisational Portfolio for the Care Home to support management of change in the Care Home to promote excellence in end of life care. This is completed by the Champion who has managerial responsibility and is able to affect change in the organisation.

The Tutors will provide one to one support and guidance to the Care Home staff and also visit the staff in their Care Home to support the development of required portfolios

Care Home Requirements for Accreditation

  • Compliance with all criteria / conditions to complete the programme
  • Valid for 12 months and organisations need to re-accredit annually
  • Each year the organisation must arrange a visit with their Personal Tutor to demonstrate they are maintaining and improving on the standard achieved when the accreditation was originally awarded
