Description
To take a step forward in the development of integrated care, the cross-European research project called ‘SUSTAIN’ was initiated, which stands for ‘Sustainable Tailored Integrated Care for Older People in Europe’ (www.sustain-eu.org). The project was funded under Horizon 2020 from the European Commission (EC). Thirteen partners from eight European countries worked together for four years to achieve the two main SUSTAIN’s objectives:

To support and monitor improvements to established integrated care initiatives for older people living at home with multiple health and social care needs, and in so doing move towards more person-centred, prevention-oriented, safe, efficient, and coordinated care;

to contribute to the adoption and application of these improvements to other health and social care systems, and regions in Europe.
The SUSTAIN consortium is glad to announce the SUSTAIN Final Conference, which will take place on Wednesday, 13th March 2019 at the European Economic and Social Committee (Room JDE62). You are warmly invited to take part in this one-day event in Brussels, Belgium.
During the conference, the SUSTAIN project will be presented in all its aspects: the research carried out within its framework will be showcased; the integrated care sites which took part in the project will be introduced; and the achieved results and project outcomes aimed at delivering better integrated care for older people, including the SUSTAIN Roadmap for Integrated Care.
