We are running this workshop to discuss the how you will manage your D2A model in the future when national funding is no longer available.
About this event
This will be a chance to share your D2A processes with other local authorities and hopefully learn how other areas are currently planning their care models once national D2A funding is no longer available.
Health and social care systems used the Government’s national discharge fund to support the best outcomes for people leaving hospital, further reduce the length of stay of acute admissions and ensure a higher proportion of people were discharged on the day that it is determined they no longer need the support of an acute hospital. The focus of the workshop will be to discuss "what happens now?"
We are deliberately keeping numbers of attendees low to allow everyone to join in the conversation and share best practices.
As part of this discussion we will look to do some high level mapping of how you plan to fund D2A beds within your organisations and highlight the pain points you are experiencing.
Once the workshop is finished, we will share all of the outputs of the process mapping, so you have something to take away and share internally with your teams.
