Thrive Trafford VCSE Strategic Forum - Trafford's Health & Social Care Transformation Plan - Improving the Health and Well Being of Trafford’s Residents.

NHS Trafford Clinical Commissioning Group and Trafford Council are developing integrated ways of working to help transform the health and well-being of Trafford’s residents. This is an opportunity for the VCSE sector to develop its vision for the transformation of health and well-being in Trafford, and work as an equal partner in planning, delivering services, and helping to reduce levels of demand.

Come along to this next important Trafford VCSE Strategic Forum event and learn more from partners about the new Transformation Plans and have your input into these important developments.
