Towards a citywide care home scheme

This half day event aims to engage with stakeholders involved in commissioning or providing health and social care to care home residents in Leeds.
All three CCGs in Leeds currently commission an enhanced care home scheme. Whilst there are some similarities in the schemes, there are also some key differences. This workshop will inform the move towards a single, citywide scheme from April 2018. At the workshop we will be exploring the learning from the current schemes, reviewing national vanguard models, exploring what a successful citywide scheme should look like, and identifying and prioritising key components of the future scheme. All outputs from the day will be used to inform a citywide business case.
We very much hope that colleagues from GP practices, Leeds Community Healthcare NHS Trust, Leeds Teaching Hospitals NHS Trust, Leeds & York Partnership NHS Foundation Trust, Leeds CCGs, Adult Social Care, Care Home providers and the voluntary sector will join us to help inform the design and development of a citywide model.
