Event Outcomes:
To provide an update on Transforming Care Partnerships and the Learning Disability Mortality review across Yorkshire and Humber 

Focusing on outcomes for individuals, provide the opportunity for professionals from across the region to share progress, best practice and challenges across Transforming Care Partnerships so that learning can be shared 

To agree regional priorities and ways of working at local, sub regional and regional level 

To learn from national best practice and gain useful practical help in tackling the issues facing local delivery and improvement
