Transforming Care through housing for people with learning disability and/or autism in London

This event will discuss the delivery of housing options to transform care for people with a learning disability and/or autism. It will bring together a range of stakeholders from commissioners, providers, and the NHS, to discuss how Transforming Care Partnerships can effectively and efficiently deliver the housing required for people with learning disabilities and autism, in a manner that meets the needs of all stakeholders.

The purpose of the event is to provide a forum for discussion and sharing of good practice and learning, away from procurement and other pressures that can inhibit open discussion in more formal settings. In a peer-to-peer environment, we will discuss and seek solutions in the following key areas:

  • Sharing approaches to strategically deliver housing needs over the long term

  • Exploring how Transforming Care sits within broader housing and investment issues in London

  • Accelerating movement out of hospital through access to housing options

  • Ensuring a smooth pathway from child to adult services, and as people age

  • Collaborative problem solving to put in place the right accommodation and support at the right time

This event will seek to discuss these questions in a practical manner, using examples of where positive outcomes have been achieved, to share learning and good practice. There will also be breakout sessions for more in depth discussion in smaller groups, and a chance to network with other attendees.
