Transforming Community Neurology: Community Neurology Project Conference

The NHS Five Year Forward View sets out a plan for patient-centred, integrated care which is delivered locally. This has major implications for people living with neurological conditions and those who provide services for them.
The Community Neurology Project is facilitating a co-ordinated programme of work to improve care and outcomes for people living with neurological conditions. The community neurology report has now launched and can be found at http://bit.ly/1tolWrk
The conference will outline the content of the commissioning brief, cover developments in community neurology and finish with workshops to decide future directions and groups to initiate action and secure funding.
