About this event

The conference has three key aims:

• To deepen understanding of the transition process to adult social care and adult health services

• To deepen understanding of the support options available for young people and their families once they transition to adult services.

• To help identify what we may need to do more of, less of, or differently, to better meet the aspirations of young people and their families.

Participants can choose to attend three workshops from a total of five: housing, health, daytime opportunities, employment and education.

Workshops must be booked in advance.

Parentsactive is the Parent Forum for Hammersmith and Fulham and is an independent voice of parent/carers. The forum encourages parents to participate in all issues relevant to disability. We also work with the Local Authority to help shape services for disabled children/young people and ensure that their needs are met.
