Places are 795 (plus VAT) per person, and include complimentary coffee, tea, water and lunch each day.

If you would prefer to be invoiced through a Purchase Order, rather than credit card, please select that option when you place your order.

Each delegate will also receive a free half-day RCA consultation at a mutually convenient time with our director Mr Gary Tyne CRL.

Who is the course for?

  • Managers responsible for process and quality improvements

  • Quality Managers, Internal and External Auditors

  • Line managers and supervisors involved in managing customer and/or supplier relationships

  • Problem solving team members and leaders

  • Product or process designers

  • Industrial, production and process engineers

  • Maintenance Technicians, Engineers and Managers

  • Health and Safety Managers

What will delegates learn?

  • Proactive versus Reactive RCA’s – Understand the difference between a proactive approach to root cause analysis, a reactive approach, and the benefits of each.

  • Trigger Points and Risk – Determine the triggers of an RCA investigation in your organisation, and what levels of risk will determine the analysis to be performed.

  • Timeline – Learn how to develop a timeline and identify what information needs to be captured to support the analysis.

  • Incident Investigation – Learn how the RCA methodology integrates with your Incident Investigation process.

  • Impact Statement – This is the problem starting point, and is the “what”, “when”, “why” and “impact” of the problem within the organisation.

  • 5 WHY Analysis – Understand the 5 WHY process with some interactive exercises that demonstrate this as the basis of cause and effect.

  • Cause and Effect Analysis – Understand the principles of cause and effect and how this can be used on more complex issues that lead to the identification of effective solutions.

  • Evidence – This will highlight the importance of evidence and look at the different types of evidence on offer that support the RCA investigation.

  • Solutions – The purpose of an RCA is to identify effective solutions and prevent reoccurrence of the problem, here we look at how to identify an effective solution.

  • Reporting – With the analysis complete, how do we communicate our findings in a simple but effective way? Here we look at various report templates and discuss the importance of tracking the outputs of the investigation.

  • Final Exercise – Here the attendees will work in teams of 4 – 5 people and complete a full investigation utilising the learnings from the training.

What value does structured Root Cause Analysis bring to an organisation:

Root Cause Analysis brings value to any organisation by bringing causes together from various sources and linking them into a logical ‘big picture’ relationship which is supported by evidence. The Root Cause Analysis will provide a point of focus, remove internal politics and allow for ongoing review and analysis. The identified solutions will be clearly seen and the investigation will become part of corporate memory. Root Cause Analysis will improve your bottom line in terms of safety, environmental compliance and company profitability.
