This is an online application for reasonable adjustments/ facilitation for the next UNISON South West Regional Council meeting.
Following the latest Covid updates and guidelines, the next UNISON South West Regional Council meeting is now being held online instead of in person.
DATE AND TIME: Saturday 29 January 2022. 10am to 2pm with breaks as needed.
VENUE: Online. MS Teams. Details will be sent to delegates registered for this meeting at least a week before the meeting is due to take place.
Anyone attending a CNR, South West Committee or Strategic Committee meetings should also use this form.
This form should be completed and submitted by the closing date, Monday 17 January 2022, and should be used in conjunction with the branch delegate or the Ex-Officio registration form.
Reasonable adjustments cannot be guaranteed after this date.
