UNISON South West Regional Council - Reasonable Adjustments or Facilitation

This is an online application for reasonable adjustments/ facilitation for the next UNISON South West Regional Council meeting.

VENUE: County Room, Colin Atkinson Building, Somerset County Cricket Club, Taunton, Somerset, TA1 1JT

Anyone attending who may need reasonable adjustments or facilitation to ensure they can attend the meeting, should complete this form as soon as you/ they've been registered to attend the meeting.

Branches should ensure that they forward this registration link to the member to complete or they can complete this form on behalf of the member.

This form should be completed and submitted by the closing date, noon on Tuesday 6 September, and should be used in conjunction with the Branch Delegate, Observer or the Ex-Officio registration form.

Reasonable adjustments cannot be guaranteed after this date.
