Event Overview:

As part of Greater Manchester Academic Health Science Network’s Patient Safety Collaborative Programme, this one day course is designed to support improvements in patient safety across your organisation.

What is root cause analysis?

Root cause analysis (RCA) is a process of investigation used after incidents to understand how and why something happened, and to learn lessons to prevent re-occurrence.

Who is this event aimed at?

This would benefit those who are leading and influencing patient safety and quality improvement in their organisations, including:

  • Patient safety/quality managers and/or leads
  • Incident investigators
  • Senior clinicians
  • Middle managers

What will attendees learn at this event?

This session will help you to:

  • Understand how teamwork, tasks, equipment, workspace, culture and the organisation can affect performance, and how this impacts on patient safety
  • Use this knowledge and awareness during investigations of incidents
  • Utilise tools to support in-depth analysis and more effective recommendations following RCA

This event is led by Jeff Goulding, Assistant Director of Human Factors at Aintree University Teaching Hospitals NHS Foundation Trust.

Registration is from 9:00 am and the start time is 9:30 am.

Many of our events operate waiting lists so this will enable us to reallocate your place to someone else on the list.
  4. We take photographs and occasionally video/live stream at our events. We use these pictures and video on our website and in other media such as Twitter and news publications. If you do not want to be photographed or filmed please let a member of the AQuA team know at the event.

If you do not want to receive information from us, please let us know by sending us an email to AQuA@srft.nhs.uk and we will remove your name from our mailing list.
  6. Supporting resources, presentations and other materials from the event will be made available on our website after the event for 30 days.
  7. By registering for an AQuA event you consent for us to share your attendance information with your organisation for reporting purposes.
