About this event

RELATIVES /HOUSEHOLD MEMBERS: If you arrive for your swab without completing and returning the necessary consent form and emailing it to this will result in the swab not being done on the day and the line manager of the staff member will be notified.

STAFF: You will be asked to complete a consent form when you attend for your swab.

Please be on time for your appointment. Due to the number of users possibly being swabbed, we ask that you stick firmly to your appointment time. Try not to be early or late.
