Work and Health: Using Personalised Care Strategies to Support Behaviour Change and Outcomes

A CPD-certified training programme, honored by the Princess Royal Training Awards, designed for professionals supporting people with disabilities or long-term health conditions to return to, remain in, or start employment. The training equips staff with practical skills to apply Personalised Care principles and proven strategies to achieve better outcomes.

Why This Training Matters

Long-term sickness is now the leading cause of economic inactivity in the UK, with 2.6 million people affected and rising disability benefit claims, particularly among young people. Poor management of long-term health conditions, most commonly mental health and musculoskeletal issues, continues to create barriers to work, costing the government an estimated £50 billion a year and limiting opportunities for individuals to live healthier, more independent lives.

While many programmes exist to help people into employment, frontline staff often feel underprepared and lacking confidence when supporting individuals with complex health needs. This can limit their ability to have effective conversations and influence positive outcomes.

Our Solution

At Pathways CIC, we have a proven track record of delivering high-quality, personalised support that helps people to better manage their health, improve wellbeing, and where appropriate, move closer to or into sustainable employment. Through a range of programmes – from early intervention services that help people remain in work, to award-winning social prescribing models that empower individuals to focus on “what matters to me” – we work in partnership with health, employment, and community providers to create meaningful, lasting outcomes. With a wealth of experience, specialist teams, and a compassionate approach, we are committed to enabling people to live healthier, happier lives while reducing health inequalities and supporting local systems to thrive.

Our training is designed to harness our experience and expertise to equip frontline staff with the knowledge, skills, and confidence to engage more effectively with people who are economically inactive or off work sick. By applying personalised care and a holistic, biopsychosocial approach, learners will leave equipped to:

Confidently apply Personalised Care principles to help individuals take ownership of their health and wellbeing, leading to improved condition management and independence.

Use effective behaviour change techniques to have more meaningful conversations that build trust, strengthen relationships, and influence positive outcomes.

Engage and motivate ‘hard-to-reach’ groups with greater confidence, reducing disengagement and improving overall programme performance.

Increase resilience and confidence in handling complex cases, helping staff feel supported, motivated, and satisfied in their roles.

Manage caseloads more effectively by using practical tools and strategies that not only support participant wellbeing and progression towards sustainable employment, but also drive improvements in engagement, retention, and overall contract outcomes.
