Working with families - Transition towards ‘outstanding’ outcomes
Event Information
About this Event
Joe Holmes is the Operations Manager for Alexandra Homes, which is a privately run residential service for adults who have an Autism Spectrum Disorder (ASD) and co-occurring mental health conditions. Our Ash View House service in South Gloucestershire is rated ‘Outstanding’ by CQC and achieved outstanding in the domain of safe.
Joe has 10-years of experience of working closely with adults at risk, their families, and professionals. Outside of work he is currently studying towards a BA (Hons) degree in Health & Social Care. Formerly, he was a positive behaviour support (PBS) and safeguarding trainer and has a passion for empowering and helping others towards their identified outcomes.
Joe will talk through and provide examples of our approach to transitioning. Transition is often difficult for people on the autism spectrum and their families, and so requires a carefully considered and measured approach to promote an effective outcome for the person.
Joe will explain their approach to admissions by talking through how they consider preparation, personalisation, and person-centred planning, together with input from professionals and family.
