Workshop: the future of patient safety investigation

Registration will open at 9.30, with the day starting at 10.00 and finishing by 16.30.

Through its Future of patient safety investigation engagement programme, the NHS Improvement patient safety team is seeking views on how Serious Incidents should be reported and investigated in the NHS.

To support this work, we are hosting four workshops, bringing together people associated with patient safety to help us further explore issues surrounding the quality and effectiveness of patient safety investigation and to jointly consider changes to systems, processes and behaviours that would improve investigation. This will inform the revision of the Serious Incident Framework.

Aims:

to allow participants to openly explore problems associated with serious incident management
to build collective knowledge to support joint analysis of these problems
to generate recommendations and solutions which can improve the quality of investigation so that it is used as a tool for learning and improvement

Who should attend:

healthcare staff (including medical directors, directors of nursing, risk management teams, patient safety managers, quality governance leads, investigators)
commissionioning, regulatory/oversight teams
professional regulators
members of other arm's length bodies
patients, families and public representatives

Places are limited to two per organisation/regional team and a proportion of places have been allocated to enable representation of different groups. Please ensure the most appropriate representative attends on behalf of your organisation.

If you have booked a place and can no longer attend please let us know at the earliest opportunity so it can be offered to someone else. If allocated spaces are not filled seven days before the event these will be given to anyone on a waiting list on a first come, first served basis.
