Young Adult Carers Transition Training

From December 2020, young carers or young adult carers who will continue in their caring role or have significant needs post 18th birthday, will need to be transitioned from Early Help 0-18 (EH 0-18) /Children’s services (CSC) to Adult Social Care (ASC) or Special Educational Needs & Disability (SEND).
The council has developed a transitional protocol which has been integrated onto MOSAIC. The process will enable EH 0-18/ CSC (young carers) to be directly transferred to ASC/ SEND (young adult carers), so they can continue to receive support from the council.
The council will be delivering a number of training sessions to assist and provide guidance to EH 0-18/ CSC front line practitioners and managers who are currently supporting young carers aged 18 and under.
The training will cover the following points:
• Importance of supporting informal carers
• What constitutes a young carer
• Care Act 2014 (section 63-64)
• Mental Capacity and how this relates to young carers
• Eligibility for adult carers under the Care Act 2014
• WF Transitional Protocol/ pathway and steps
• Importance of interagency working
• Practical demonstration/ guidance of the workflow, steps and transition between services (MOSAIC)
We encourage staff and managers who are supporting young carers to join one of the 2 sessions so that you are equipped and ready when the protocol and systems go live.
