We want to see empowerment and voice for all residents of Wormholt and White City, in particular for excluded groups such as young people, people with English as a second language, people living in poverty, and people living in isolation.We would like to see access to services for everyone, delivered appropriately according to their needs and culture.

Parkview Centre for Health & Wellbeing

<p>W12 Together, the NHS Primary Care Network and LBHF Family Champions are working together to hear from local residents about their experiences of GP services in North Hammersmith &amp; Fulham.</p><p>We are inviting residents who live with a long-term or ongoing health condition to take part in a friendly, small focus group to share what is working well and where GP services could be improved. Your feedback will help shape how local primary care services are delivered in the future.</p><h3><strong><strong>Who this session is for</strong></strong></h3><p>Residents living in North Hammersmith &amp; Fulham who:</p><ul><li>Live with a long-term illness, ongoing condition, or disability, and</li><li>Attend GP practices in the White City, Wormholt or Old Oak areas.</li></ul><p>You do not need to share personal medical details to take part — just your experiences of accessing care.</p><h3><strong><strong>Why take part</strong></strong></h3><p>Your voice matters. People living with long-term conditions often have the most ongoing contact with primary care services. By sharing your experiences, you’ll help:</p><ul><li>Identify what is working well</li><li>Highlight what could be improved</li><li>Shape how local GP services support residents in the future</li></ul><p>This is part of a wider programme of engagement across the community, and your contribution will help make local healthcare more accessible, compassionate and responsive.</p><h3><strong><strong>Session Details</strong></strong></h3><p>Date: Tuesday 2nd December 2025<br>Time: 10am to 12pm<br>Location: Community Room 2, Parkview Centre for Health &amp; Wellbeing<br>Duration: 2 hours<br>Refreshments provided</p><h3><strong><strong>Accessibility</strong></strong></h3><p>If you have any accessibility needs, please let us know when registering.
