EHC Plan School Transition - Post-16

We are having this session to support young people aged 16-25 with a EHC Plan and their parents/carers, transitioning into new post-16 educational setting in September 2026.

By the 31st March 2026, you should receive a finalised EHC Plan for your transition into your new post-16 educational setting school.

These information sessions from the SIAS Team will offer -

• What steps to take if you are not happy with the type/name of school listed i.e. mediation and appeals
• General support to help the transition process
• Dicussions around school settings

The session will be held online via teams.
