Supporting People living with Dementia and Frailty - 1 day Masterclass
This Masterclass will equip you with knowledge, skills, tools and techniques to improve care within your organisation. The teaching will use a variety of methods. The approach is of interactive and inclusive learning.
Learning Outcomes (Mapped against Tier 3 of the Knowledge and Skills Framework (2015)
Recognise the dynamic nature of frailty
Understanding assessment and the impact of co-morbidity
Appreciate the essential elements of co-ordinated care
The content includes:
Knowledge of current policy, research and evidence, covering key documents and exploring the concept of frailty as a syndrome.
The application of frailty assessment tools within a wide variety of settings and the barriers to these within own practice.
Understanding the complex relationship between frailty and dementia as co-morbidity.
Predicting and pre-empting crisis through health promotion and professional role.
Co-ordinating and planning multi-professional care in order to manage and support people with frailty syndrome.
Using clinical case studies to inform practice and develop an action plan
Any health and social care staff involved in the end of life support of somebody living with dementia. A certificate of attendance will be awarded at the end of the workshop.

*All students employed by a Worcestershire NHS Trust or Wye Valley NHS Trust have to complete a study leave form (or equivalent) if they want (a) to study in works time, or (b) want the Trust to fund or part fund the course, or both. Upon receipt of your approved trust study leave form you will receive notification that your place is booked. If you attend these days without an approved study leave form you will be charged.
In any given year the availability of conferences, events and study days will depend on delegate numbers. In the event of a conference, event or study day not being run, due to a low number of delegates registering, delegates will in these circumstances be offered a full refund; otherwise no refund of fees will be made

Is my registration/ticket transferrable?
Yes - you can amend your registration details on Eventbrite. However, after registration and payment of the workshop fee no refunds will be made unless the worskhop is cancelled due to insufficient delegates registering.
