Patient Flow Hack - Sheffield

The Yorkshire and Humber AHSN is one of 15  innovative health networks set up to create and harness a strong, purposeful partnership between patients, health services, industry, and academia.The aim of the Yorkshire & Humber AHSN is to create significant improvements in the health of the population by reducing service variability and improving patient experience in the health care system.  The Yorkshire & Humber AHSN will assist in ensuring new innovative products and services that have the potential to transform lives become part of routine clinical practice.  The Yorkshire & Humber AHSN will also assist in providing economic growth for the region, supporting inward investment projects and industry that support the health sector.

What is a Hack? 
A ‘Hack’ is based on the traditional concept of a ‘Hackathon’, whereby we bring people together and use their combined knowledge, wisdom and expertise to find new solutions to old problems over an intensive period of time.
What is the problem? 
The Patient Flow Hack is designed to provide a tangible solution to assist the challenge of patient flow. Patient flow is a complex and difficult challenge for all organisations. Colleagues from different departments from across Sheffield Teaching Hospitals NHS Foundation Trust will explore together the patient journeys. Real data and patient journeys will be used to develop a shared understanding of patient flow throughout the Hospital, which means that we can collaboratively identify where we should focus our efforts to make improvements.
Question we will address - How can we optimise the patient journey through the health care system to improve patient flow?
As a result of this event a series of projects will be developed with the following aims:
• Identity solutions to improve acute care • Address methods to help us work towards more timely and effective discharge
What to expect? 
Local data from the last year will be used to model real patient journeys through the hospital from admission to discharge. The event will start with a physical simulation using this data to develop a shared understanding of the challenges and bottlenecks relating to patient flow. Real patient journeys will be used and will model a typical good day and a typical bad day in the A&E department. 
The event brings together colleagues, academics, senior managers and front line staff to understand patient flow challenges from across the system. This event will create a sense of energy and excitement to tackle a really difficult challenge. Please come along, share your experience and expertise and help us to hack the problem. 
The Yorkshire & Humber Academic Health Science Network (AHSN) and the Improvement Academy (IA) will support the projects that are identified as a result of hack.
Should i attend?
It is important that we have a good representation from across the hospital to ensure we have a full systems perspective. Please see examples of the range of attendees we would like to see at the Patient Flow Hack. Please note, this is not an exhaustive list and all staff are welcome. 
• SRG Leads • Urgent Care Leads • Clinical staff (e.g. Consultants/Matrons /Sisters/Nurses) from A&E, Medical Assessment Units, Medical Wards and Discharge Lounges • Patient Flow Co-ordinators, Hospital Bed Managers, Operational Delivery Managers, Service Managers, Ward Managers, Discharge Facilitators and Hospital Re-Ablement Social workers • Community Matrons and Case Managers • Service Improvement Team Leads and Performance/Analysis Leads
