Are you a clinician or a healthcare practitioner? Are you looking to implement new models of care, or transform integrated care services in your local area?

We are offering a series of Open Events for other NHS providers, vanguard sites and employees from key NHS healthcare organisations to share best practice, talk through our journey so far and our plans for the future.

In the last six months of vanguard funding we are working hard to further the development and spread of our new care models at scale, across 90% of our local GP practices. Current research is beginning to show the positive impact across Yeovil Hospital's key performance measures as well as enhancing the role of our GPs and strengthening services within primary care.

Each open event will feature key staff who have been instrumental in developing and leading the different workstreams from within our vanguard site and will offer an opportunity to ask questions, share your own learning and explore opportunities for future development.

Draft programme will cover:

• Background to the Symphony Programme, initial research, programme positioning and focus
• Introduction to primary care operating company: Symphony Healthcare Services Ltd
• - current situation,new GP roles, integration process and contracting arrangements
• Introduction to New Care model developments across south Somerset
• - support for the most complex patients
• - new roles in Enhnaced Primary Care that support both patients and GPs
• - improving the specialist training and resources being developed in primary care
• - spread, impact data and future vision
• Update on the Symphony Programme vanguard
• - commissioning arrangements, joint venture and link to STP

The programme will start with tea/coffee on arrival and introductions between 13.00 and 13.30pm and will conclude around 17.00, with an opportunity for an informal Q & A between 16.30 and 17.00pm
