<p>This workshop will be particularly helpful for parents and carers who have a child transitioning to secondary school or any other change in school or college setting. </p><p>We will explore tried and tested tips to help a smooth transition and use Person Centred Planning tools to explore different ways for you to plan transition and identify the things that are important for your child’s transition.</p>
