The Jean Bishop Integrated Care Centre in Hull aims to keep residents fit, out of hospital and independent in their own homes. People attending the centre will have an integrated care plans formed around them and a coordinator appointed on the same day, with a variety of services including: therapists, voluntary services and others based under one roof. This is a multi-disciplinary approach across services and focused on prevention.

The approach of the new centre dovetails with the vision of the regional ADASS integration network intentions in seeking to share good practice and grow a culture of collaboration across organisations in delivering person centred care.

The Integrated Care Centre in Hull was identified at the initial regional integration workshop as a new provision embodying an integrated approach. This session in Hull is an opportunity to observe their integrated approach in action and be part of a regional conversation, learning together about how we can together work to improve outcomes for residents across the region.

Hull's intentions to strengthen relationships across system partners, has enabled them to think differently and create opportunities that have made a positive impact for people who live in the city.
